1. Has a GPA of 3.0 or higher on a scale of 4.0.
2. Is enrolled in or has been accepted to a college or university for the 2020-2021 academic year.
3. Has a financial need.
4. Plans to attend school in the state of Virginia.
5. Has completed all prerequisite courses required for your degree.
6. Maintains a permanent residence in one of the following counties: Albemarle, Alleghany, Amherst, Appomattox, Augusta, Bath, Bedford, Bland, Botetourt, Campbell, Carroll, Craig, Dickenson, Floyd, Franklin, Giles, Grayson, Halifax, Henry, Highland, Lee, Montgomery, Nelson, Patrick, Pittsylvania, Pulaski, Roanoke, Rockbridge, Rockingham, Russell, Scott, Smyth, Tazewell, Washington, Wise, Wythe.
7. Is motivated to achieve your education and career goals.

1. The application must be completed and submitted online.
2. The attachments and other documents must be mailed to the Health Focus of SWVA P.O. Box listed on the application summary page.
3. Please review the list of required documents now, so you have ample time to make the appropriate requests for transcripts, etc. (Postmark deadline is May 23, 2020.)
